The Community HealthCare Association of the Dakotas (CHAD) and the Great Plains Health Data Network (GPHDN) thanks you for attending the 2024 CHAD/GPHDN Annual Conference “IMPACT: The Power of Health Centers.” This annual event connected leaders like you from community health centers across Wyoming, South Dakota, and North Dakota to come together.

​​This year’s conference was packed with informative sessions on building culture, strengthening your workforce, emergency preparedness, integrated behavioral health care, and using data to advance the health center program. Additionally, two pre-conference workshops were offered specifically for workforce development and emergency preparedness.

IMPACT: Workforce Strategic Planning Workshop

Presenters: Lindsey Ruivivar, Chief Strategy Officer, and Desiree Sweeney, Chief Executive Officer

It’s time to get strategic about workforce! This pre-conference workshop kicks off a workforce strategic planning series led by NEW Health, a community health center serving rural northeast Washington State. NEW Health developed its robust workforce development plan called NEW Health University after many years of developing creative solutions to rural workforce challenges. NEW Health believes if their rural, resource-limited organization can develop a comprehensive workforce development plan, any health center can!

IMPACT: Emergency Preparedness - Trauma-Informed De-Escalation and Incident Management

Presenter: Matt Bennett, MBA, MA

This in-person workshop is designed for healthcare professionals and leaders at health centers seeking strategies for managing confrontations with angry, retraumatized, or frustrated patients. Participants will learn to de-escalate hostile situations, ensure safety, and enhance patient care quality. The workshop encapsulates the principles of trauma-informed communication, enabling professionals to understand and respond empathetically to patients who have experienced trauma.

Power of Culture
Presenter: Vaney Hariri, Co-founder and Chief Culture Officer

A better culture is better for everyone. Vaney Harari from Think 3D kicks off our annual conference with a keynote address that dives into the critical impact that organizational culture has on an organization and its people, teams, and resources.

Attendees should be prepared to examine their definition of workplace culture, be willing to look at what they are (or aren’t) contributing to that culture and expect to walk away with at least ONE actionable plan for elevating their culture.

Power of Culture works through simple but fundamental shifts in perspective that help organizations, teams, and leaders understand the importance and benefits of investing in a healthy, positive, and productive organization. When we align on what that culture should look like, we can move towards it more effectively.

Amber Brady will discuss the challenge of access to healthcare in rural North Dakota and what Coal Country Community Health Center is doing to address their communities’ needs. Why Primary Care Behavioral Health?

Presenters:  Bridget Beachy, PhysD, and David Bauman, PhysD

The lack of access to mental health treatment continues to plague the United States healthcare system. Further, decades of research have demonstrated that primary care continues to be the “de facto mental health system.” These realities have led to innovations and efforts to integrate behavioral health providers into primary care. This presentation will provide an overview of the realities of mental health treatment in the United States and provide a rationale for integrated behavioral health models that focus on increasing access to care. The presenters will share information about the Primary Care Behavioral Health model and alternative approaches to delivering behavioral health treatment to reach communities.

Specifically, presenters will allow attendees to explore their values and whys related to their behavioral health profession and how adopting a moment-at-a-time approach may enhance these true values. Further, attendees will learn strategies and philosophy shifts that allow a moment-at-a-time approach to make sense and provide care that is not only accessible but radical, compassionate, and engaging. Lastly, attendees will have time to practice the skills they learned through role-plays to enhance their comfort, confidence, and comfort in delivering care from a moment-at-a-time philosophy.

Communication is more than reading, writing, and speaking – it is a skill for effectively transferring information and influencing behavior change. In this two-part session, attendees will review key principles of effective communication, core challenges and identify key opportunities for improvement.

The session will introduce Think 3D’s POWER communication and coaching model. The model outlines best practices for giving and receiving feedback, developing clear expectations for communication and coaching from leaders, and the POWER communication method.

By the end of these sessions, attendees will better understand how to improve their communication skills, overcome common communication challenges, and effectively influence behavior change.

Furthermore, the session will discuss the importance of continuous quality improvement efforts in ensuring the delivery of great care within the healthcare system.

Role of Behavioral Health in High-Quality Primary Care

This session will detail how integrating behavioral health providers fully into primary care allows health systems to answer the call set forth by the National Academies of Sciences, Engineering, and Medicine (2021) to implement high-quality primary care. Specifically, the presenters will detail how the Primary Care Behavioral Health model’s goals align parsimoniously and effortlessly with the goals of high-quality primary care. Further, the presenters will detail how integration care efforts go beyond solely treating behavioral health concerns in primary care. Lastly, data from a community health center in the state of Washington will be presented to reinforce how the PCBH model has moved the CHC closer to the infinite values of high-quality primary care. This session is appropriate for all healthcare team members, including executive leaders.

Quality improvement data is critical in identifying health disparities and implementing effective solutions to address them. In this session, Shannon Nielson will introduce health centers to the foundations of building an equity strategy within their existing quality program.  Attendees will discuss how to define, measure, and improve equity across clinical quality measures. The session will include an introduction to an equity scorecard framework, and health centers will learn how to utilize health equity data to drive a systems culture of equity. Attendees will also be introduced to strategies to improve the reliability of health equity data from collection to reporting.

SELF – is an acronym that stands for Support, Ego, Learning, and Failure. The session will walk through how those principles can be utilized to reflect on your personal development and identify opportunities to become a better you!
