Celebrating Success, Looking to the Future

THE HEALTH CENTER JOURNEY

Health Centers in the Dakotas are connected to a robust and proud history of providing high-quality health care for decades. The 2021 CHAD and Great Plains Health Data Network (GPHDN) Conference, The Health Center Journey: Celebrating Successes, Looking to the Future, was held virtually on September 14 & 15. The conference kicked off with a keynote address, sharing a historical look at the health center movement, followed by a panel of recent retirees who shared stories about how they’ve seen and impacted significant growth in health centers in their collective time of over 100 years working in the health center program. Another session discussed how we could improve health outcomes and overcome social determinants of health that tribal communities face with an understanding and respect for cultural resources and a focus on community empowerment. Following sessions focused on clinical quality and health equity, behavioral health, health data strategy, workforce engagement, and social determinants of health. How do we lean into the legacy of the health center movement in our current moment? This session tied together key themes of the conference through one health center’s story of understanding and responding to patients’ social determinants of health (SDOH). In this engaging presentation, Ms. Francis shared how health centers can use the PRAPARE tool, including the opportunities and challenges of implementation and how that data can identify disparities in clinical measures and vaccine penetration.

Health Center Spotlight: Addressing Social Determinants of Health

During this interactive panel discussion, health center staff discussed successes in effectively responding to patients’ social determinants of health, spotlighting strategies for securing strong staff buy-in for PRAPARE implementation and examples of how the integration of social work into care enhances one health centers’ capacity for addressing social needs. Panelists shared examples of providing patient-centered care to LGTBQ individuals and people experiencing homelessness, as well as tangible strategies for addressing food insecurity.

Using Data Analysis to Drive Health Equity: The Health Center Experience

Speaker: Zachary Clare-Salzler, Data Analyst and Reporting Coordinator, Partnership Health Center

Mr. Clare-Salzler shared how Partnership Health Center (PHC) uses social determinants of health (SDOH) data analysis to drive health equity. Attendees heard a review of the health center’s PRAPARE data collection strategy and how PHC has effectively integrated community health workers (CHW) into its model of care. He shared his experience utilizing the Azara PRAPARE module for SDOH data analysis, including how this data overlays with clinical quality measures. Mr. Clare Salzler also shared an example of equity-lens reports on health center COVID-19 vaccination data.

Speakers Dr. Beachy and Dr. Bauman provided a brief overview of the primary care behavioral health (PCBH) model of behavioral health integration, a nationally recognized model that the Department of Veterans Affairs currently uses. They discussed therapeutic assessment, case conceptualization, and brief interventions using fACT and other modalities commonly used in PCBH. The speakers familiarized participants with the concept of functional contextualism and how it can be used to help providers fulfill their role within the PCBH model of care.

Focusing on workforce engagement, Ms. Dixon-Foley shows that no two organizational cultures are the same. Individual make-up, organizational structures, and departmental expectations can vary greatly. In this presentation, the speaker provides concepts and practices that will help health centers see more effective workplace cultures, improved performance, better patient outcomes, and better retention and recruitment.

Building a Data Strategy for Long-Term Success

This session provided attendees seven key steps of building a data strategy that will lead to the successful implementation of Azara as well as introduced a hands-on workshop curriculum that will be provided to ensure each health center has a solid data strategy that can be used within their organization.
