The Great Plains Health Data Network Summit (GPHDN) will feature national presenters who will share their health data success stories, lessons learned, and ways health centers can work together through a health center controlled network (HCCN) to optimize health technology and data. During the morning, speakers will outline the challenges and opportunities of virtual care, and they will lead health centers in a workshop discussion of how virtual care might align with health center strategic goals. The afternoon will focus on capturing data and conducting data analysis – including what the GPHDN has accomplished so far and where it might consider heading next. This event will culminate with the GPHDN strategic planning, and it will result in a new three-year plan for the network.

After two years of seemingly unending upheaval, it’s time to reflect on the past and imagine a new future that we build together. As health care leaders, we have the opportunity of a generation to evaluate where we are now and plant the seeds that will sprout into the future of the next 10-15 years of health care. 

In this session, we’ll explore what we’ve learned from the COVID-19 pandemic and what we’ve learned from previous leaps forward in technology. We’ll compare the lessons learned from companies like Borders and Kindle, Blackberry and Apple, and Blockbuster and Netflix to increase our readiness to lead the charge on change. We’ll take this learning to catapult us into the future of what our organizations, our communities, and our nation might look like in the future and examine how we will play a role in shaping the future.

Using little more than a shoestring and a piece of gum, health centers across the country implemented virtual care in record time in 2020. But two years in, as we return to “normal,” our gum is losing its stickiness, and our shoestring is looking worse for wear. Health care leaders are at a decision point: do we invest the time, energy, and money into making awesome virtual care part of our everyday practice, or do we bet that most patients and providers will stick with the old way of in-person visits? 

Leapfrogging into the future, this session will delve into the big picture changes that virtual care brings to the health care landscape. We’ll answer burning questions like, what’s in it for our patients and staff? How can virtual care help us to meet our data strategy goals? How can we get paid to provide virtual care? This session will explore concrete examples of actual community health centers’ work in virtual care, jumping-off points to decide if virtual care is worth the effort, and where to go from here.

In this activity, we’ll begin to align our virtual care activities with the organizational goals you identified in your recently developed data strategies or other strategic priorities.

We look forward to sharing our vision for the Great Plains Health Data Network in this session. We’ll also present the highlights of the Azara project and discuss how we can use data to drive change as a network.  

2:15 PM – 3:00 PM  | Michigan Primary Care Association (MPCA) Experience with Azara
Cheryl Gildner, MA, Data Manager, Michigan Primary Care Association 

We will provide an overview of how MPCA is maximizing the use of Azara with its members, including through the use of health center registries and dashboards. You will learn how data can be used to impact specific quality metrics and improve patient care.

3:15 PM – 4:15 PM  |  Risk Stratification with Azara

This session will include an overview of the Azara risk stratification tool that will be implemented at the network level. We will explain the risk stratification timeline, the risk criteria detail, and how to validate the data to understand the risk.

During this time, you will have an opportunity to reflect on what you have learned and to pose any remaining questions.

Safety Precautions

Member-facing conference and summit events 

CHAD is committed to providing a safe, productive, and welcoming environment to learn. As of March 1, 2022, the Centers for Disease Control and Prevention (CDC) launched new guidance regarding COVID-19 community levels. As a result, wearing a mask will be determined based on the most recent data from the CDC for the county where the meeting is taking place. If the county is high-risk, masks will be required. The CDC recommends that any large indoor gathering attendees complete an at-home rapid test as close to the event as possible regardless of vaccine status to help reduce the risk of spreading COVID-19. CHAD highly recommends all attendees follow that recommendation. We recognize there may be people attending who are immunocompromised or may make a choice to wear a mask, and CHAD will have masks available at the event.  

COVID-19 Vaccinations 

Neither North Dakota nor South Dakota currently require proof of vaccination to enter public indoor locations. The CDC strongly encourages everyone who can get up to date on COVID-19 vaccinations to reduce the spread and severity of COVID-19 infection. 

Screening 

To mitigate the spread of COVID-19, if you are experiencing symptoms of COVID-19, such as fever, cough, or shortness of breath, we ask you to stay home. If you have come in close contact with someone with COVID-19, we ask you to follow the CDC quarantine and isolation guidance for vaccinated and unvaccinated individuals.  

Social distancing and handwashing 

One of the best ways to avoid any viral infection is through good social distancing and handwashing practices. As we begin to get together, we should be mindful and respectful of varying levels of comfortability when it comes to social distancing; for example, consider elbow bumps vs. handshakes. The venue will have multiple hand sanitizing stations available, and all are encouraged to use them upon entering the event and when leaving or returning from anywhere outside the establishment.
