CHAD hosted an information session on Nexus SD, South Dakota’s Community Information Exchange (CIE). Nexus SD is a statewide collaboration of healthcare, human, and social service providers sharing information using an integrated technology platform and referral system to coordinate whole-person care. The system centralizes resources available to address social drivers of health and allows real-time electronic referrals between service providers to address an individual’s needs.
In May, Nexus SD launched a statewide onboarding process to bring its system to healthcare, human and social service providers. Brittany Zephier, Community Engagement Coordinator with Helpline Center, provided an overview of Nexus SD, gave a brief demo of the system to review its basic functionality and provided the next steps to join.
