Session 1:
Advanced Primary Care Management: What CHCs Need to Know
April 8, 2025

Advanced Primary Care Management services are making their debut in FQHCs in 2025. According to CMS, this change will better align payments to RHCs and FQHCs for these services with other providers who furnish similar care coordination. This seems like a huge win for the FQHC, especially with the opportunity for billing of the add-on codes associated with these services. With CMS providing a transition period of at least six months to update our billing systems, there are several components health centers need to consider to ensure compliance.

In this session, we will introduce Advanced Primary Care Management (APCM) and how it relates to existing care management services. We will explore documentation and care delivery requirements, consider reimbursement rates, and discuss performance monitoring. Finally, we’ll consider current tools for care management activity tracking and determine whether additional tools will be needed.

This session is beneficial for clinical, operational and billing and coding staff.

Session 2:
Community Health Integration (CHI): A Review of CHI Services, Documentation Requirements & Best Practices
May 13, 2025

The session will introduce CMS-reimbursed Community Health Integration (CHI) services, highlighting how CHI differs from traditional care management and social work and providing practical guidance on eligibility, documentation, billing, and clinical workflow integration. Through case studies and discussion, participants will see how proactive documentation and CHI services can improve health care outcomes. Resources and tools will be provided to support implementation, including coding references, CMS guidelines, and screening templates.

This session is beneficial for clinical, operational, billing and coding staff.

Session 3:
Medicare Dental Coverage: 2025 Updates
June 10, 2025

In 2025, the CMS finalized rules expand Medicare dental coverage, including payment for dental services integral to Medicare covered services. This one-hour webinar will clarify what dental services Medicare covers, when and why they are reimbursable, and how to document and bill them correctly. This session provides practical guidance with real-world examples, documentation tips, and best practices to ensure compliance.

Session 4:
