CHAD is excited to share a new resource designed to support health centers in improving blood pressure (BP) control, a critical component of chronic disease management. In collaboration with our partners and in alignment with CHAD’s strategic goals, we have developed a set of Blood Pressure Competencies that outline best practices for accurate BP measurement and effective hypertension management in the primary care setting.
To complement these competencies, our partners at the Association for Utah Community Health (AUCH) have shared a valuable training video that demonstrates proper blood pressure measurement techniques, helping to ensure consistency and accuracy across care teams.
This resource supports CHAD’s broader goal to improve performance across five key clinical measures—A1c control, hypertension control, depression remission, well-child visits, and colorectal cancer screening—by an average of 2% across member health centers. It also aligns with the GPHDN Clinical Advisory Committee’s priority to improve hypertension outcomes. A key focus of this work is to increase blood pressure control among Black, Hispanic, and American Indian patients by 4% between 2023 and 2026.
Moreover, this work aligns with CHAD’s ongoing partnership with the North Dakota and South Dakota Department of Health, reinforcing a collective commitment to chronic disease prevention and control. We encourage all health centers to integrate this resource into staff training, workflows, and quality improvement initiatives. Accurate BP measurement is foundational to effective hypertension management.
