Foundations of Nurse-Led Chronic Disease Management
In this session, we introduced the benefits of nurse-led chronic disease management, including better patient outcomes, higher staff satisfaction, and financial gains. We highlighted the importance of trust as the key to a successful program. Participants also learned about the latest methods for delivering chronic care management, as well as the necessary activities and practices to implement these programs effectively.
This workshop is the first of a three-part series designed to help participants develop an actionable plan for starting or improving a nurse-led chronic care management program in their organization.
