2025 Diabetes Prevention & Awareness Toolkit

This toolkit provides health centers with ready-to-use materials to promote diabetes awareness and encourage patients to get screened. It includes outreach messaging, social media posts, posters, and patient handouts, along with clinical resources and education materials to support both providers and patients.

Prediabetes is like a pre-diagnosis of diabetes. It’s a warning sign that a person’s blood sugar levels are higher than normal but not high enough yet to be officially diagnosed as type 2 diabetes. Approximately 84 million American adults—more than 1 out of 3—have prediabetes. Of those with prediabetes, 90% don’t know they have it. 

Prediabetes raises the risk of type 2 diabetes, heart disease, and stroke—but lifestyle changes like healthy eating and regular activity can greatly reduce that risk.

  • Age 40+ 
  • Overweight 
  • History of gestational diabetes 
  • High blood pressure
