CHAD hosted the 2025 Uniform Data System (UDS) training sessions on October 15 and 16. These free, web-based trainings guided participants through preparing the 2025 UDS report, covering key updates, reporting requirements, and best practices.
Effective reporting relies on understanding the relationship between data elements and tables. This interactive training helped new staff understand their role in UDS reporting while offering experienced staff the chance to sharpen skills, ask questions, and learn from peers. All financial, clinical, and administrative staff are encouraged to view.

Zillehuma Ebrahim
Zillehuma Ebrahim is a curious, dedicated Muslim woman, who is a UK native living in Minnesota for the past 25 years. She serves as the COO for a group of FQHC clinics, with over 15 years of experience within different FQ’s. She also serves as a contractor with AHP, a UDS reviewer/trainer, and a HRSA Grant Reviewer.
She has worked in the FQHC world since moving to MN, working in various C-suite positions. She loves all projects FQHC and, yes, even the UDS! With many years of leadership and strategic planning experience, she gained critical knowledge through her direct participation in the growth and success of many non-profit organizations.
Her educational background is a Bachelors in Pharmacology from UCL London and postgraduate in Business Management and Finance at Thames Valley University, London. Her passion is to work and help serve the underserved, working in the non-profit sector has allowed her the opportunity to make a difference in the lives of people, creating a lasting impact.
