Leveraging Technology to Support Care Management Success
This workshop focused on steps for scaling chronic care management programs while ensuring high-quality patient care. Participants learned about the essential training required for nurses new to care management, as well as ongoing training needed for long-term success. The session highlighted the role of asynchronous, bidirectional communication in patient enrollment and care management, which is key to scaling a program sustainably. We also discussed how to leverage Azara to use population health data and risk stratification to target interventions for maximum impact in nurse-led care management programs. Additionally, we identified common risks and challenges, along with strategies to mitigate them.
