Building and Sustaining Nurse-Led Care Management Programs
This workshop provided practical strategies for creating or enhancing effective nurse-led chronic care management programs. Participants learned key concepts, such as how nurses can use risk stratification to determine patient panel size, prioritize patients, and structure their care management programs. The session also covered best practices for enrolling patients in these programs and how to create and utilize care plans to guide successful management.
Additionally, attendees explored quality improvement (QI) strategies, including starting small, testing workflows, and refining processes before expanding the program for wider impact.
