Driving Preventative Care with Data & Outreach Tools

CHAD recently wrapped up a successful three-part webinar series aimed at increasing Well-Child Visits, Adult Annual Wellness Visits, and Medicare Annual Wellness Visits. This initiative was a collaborative effort between CHAD’s clinical quality and marketing teams and utilized valuable input from the Clinical Advisory Committee. Presenters included Lisa Thorp from Quality Health Associates, Josh Brock from Family Health in Idaho, and Dr. Shawnda Shroeder from the University of North Dakota Department of Indigenous Health.  

The series emphasized the pivotal role that preventive visits play in driving quality improvement across health centers. By ensuring patients come in for their annual and well-child visits, care teams are better positioned to identify and close gaps in care, deliver timely preventive screenings, and manage chronic conditions more effectively. 

The sessions also featured practical demonstrations of Azara DRVS tools, including the use of custom registries, risk stratification features, custom scorecards, and effective measures to track and identify gaps in cancer screenings, immunizations, and well visits. These data-driven tools empower health centers to take a more targeted, proactive approach to patient care, aligning preventive visit strategies with quality goals.  

These visits are not only key to improving patient outcomes, they’re also essential to strengthening performance in value-based care models.  

As part of the webinar series, CHAD developed comprehensive toolkits designed to support health centers in implementing and sustaining efforts to increase preventive visits. These essential resources include staff education materials to reinforce the importance of well-child and annual wellness visits, along with practical tools to streamline outreach and communication.  

Health centers will find ready-to-use phone script examples to support consistent messaging during appointment scheduling, customizable patient letters for integration into the EHR, and text message templates to enhance outreach and education efforts. The toolkits also include vibrant marketing posters and social media graphics that can be customized to reflect health center’s branding, helping to consistently promote preventive care through multiple outreach channels  

Health centers are encouraged to explore and utilize the supporting toolkits developed alongside the series, which offer practical strategies, patient engagement tips, and data-driven resources to enhance outreach and care delivery.
