This presentation will present a break-even analysis tool tailored for FQHCs, offering participants a comprehensive understanding of its significance in financial management. Break-even analysis helps FQHCs assess financial viability by identifying the point where total revenue matches total costs, ensuring neither profit nor loss. Attendees will learn about the necessary components – fixed costs, variable costs, and revenue factors – and how to apply them to FQHCs. Practical strategies for utilizing break-even analysis to inform financial and operational decisions within FQHCs will also be explored.

Participants will learn key concepts, such as how nurses can use risk stratification to determine patient panel size, prioritize patients, and structure their care management programs. The session will also cover best practices for enrolling patients in these programs and how to create and utilize care plans to guide successful management. Additionally, attendees will explore quality improvement (QI) strategies, including starting small, testing workflows, and refining processes before expanding the program for wider impact. By the end of the session, participants will have a clear, actionable plan for developing or improving a chronic care management program within their organization.

Leveraging Technology to Support Care Management Success (90 Minute Session)

This workshop will focus on steps for scaling chronic care management programs while ensuring high-quality patient care. Participants will learn about the essential training required for nurses new to care management, as well as ongoing training needed for long-term success. The session will highlight the role of asynchronous, bidirectional communication in patient enrollment and care management, which is key to scaling a program sustainably. We’ll also discuss how to leverage Azara to use population health data and risk stratification to target interventions for maximum impact in nurse-led care management programs. Additionally, we will identify common risks and challenges, along with strategies to mitigate them. The session will end with a plan of action, helping participants set concrete goals to advance their chronic care management programs within the next 90 days.
