Health Centers in the Dakotas are connected to a robust and proud history of providing high-quality health care for decades. Now to be held virtually, the 2021 CHAD Annual Conference, paired with the Great Plains Health Network summit, will feature national experts and engaging speakers and panelists. Attendees will look at the history of the health center movement as a way to inform the current moment and look forward to the future’s potential.
Together we will connect to the past through stories and learn how to use storytelling to continue to be community-driven, equity-oriented, and patient-centered organizations. Using these skills, we can continue to live out the values of the health center movement in the current context.
All sessions are listed below. Conference registration is $50 per person for members and $100 per person for non-members and that cost covers all sessions, workshops, and breakouts. CHAD will invoice participants after the conference – no need to pay now. We ask that you register for ALL sessions you plan to attend. At each session, there is an option to add the event to your calendar. Please note that there are general sessions that are intended for all participants and break-out sessions/tracks for specific attendees. For questions about where and how

Kicking off the 2021 CHAD Conference, CHAD CEO Shelly Ten Napel will welcome participants, discuss the interactive and compelling two-day agenda, share some housekeeping details, and introduce our first speaker.
We know that telling the health center story has never been more important, from our COVID-19 successes in testing and vaccinations to the unprecedented funding opportunities included in the American Rescue Plan Act. In this session, Ms. Johnson Woodard will look back on the history of the health center movement as a way to inform the current moment and look forward to the future’s potential. How do we lean into the legacy of the health center movement in our current moment? This session will tie together key themes of the conference through one health center’s story of understanding and responding to patients’ social determinants of health (SDOH). In this engaging presentation, Ms. Francis will share how health centers can use the PRAPARE tool, including the opportunities and challenges of implementation and how that data can identify disparities in clinical measures and vaccine penetration. Participants will hear how they can utilize SDOH data to inform organizational strategic planning and better address the root causes of health and well-being, including introducing community organizers as health center staff.

Panelists will share examples of providing patient-centered care to LGTBQ individuals and people experiencing homelessness. Using Data Analysis to Drive Health Equity: The Health Center Experience

In this session, Mr. Clare-Salzler will share how Partnership Health Center (PHC) uses social determinants of health (SDOH) data analysis to drive health equity. Attendees will hear a review of the health center’s PRAPARE data collection strategy and how PHC has effectively integrated community health workers (CHW) into its model of care. He will share his experience utilizing the Azara PRAPARE module for SDOH data analysis, including how this data overlays with clinical quality measures. During this interactive one-hour presentation, Mr. Clare Salzler will also share an example of equity-lens reports on health center COVID-19 vaccination data.

They will discuss therapeutic assessment, case conceptualization, and brief interventions using fACT and other modalities commonly used in PCBH. The speakers will familiarize participants with the concept of functional contextualism and how it can be used to help providers fulfill their role within the PCBH model of care. This session is applicable for licensed behavioral health providers and is approved for 7 CE hours.
