National Health Disparities Collaboratives Library
Sign up as a "new user” or sign in with an existing user name and password to access the library and search documents.

The Health Disparities Collaboratives Public Library is also available and allows users to locate topics using the search button on the right hand side of the page.

Customizing CHAD Member Resource technical assistance for specific clinical quality topics is available to CHAD member staff by contacting Ann Skoglund, RN, clinical quality specialist,. A clinical quality network team has also been established to cover various topics and provide periodic trainings.

In addition, CHAD is pleased to offer access to the CHAD Member Resource OC3 Manual

The Optimizing Clinical Care Collabrative (OC3) manuals created by the Texas Association of Community Health Centers (TACHC) provide a wonderful opportunity for the health centers to have access to up-to-date policies and procedures.

Quality Healthcare Model

All community health centers in the Dakotas have implemented the chronic care model and completed training offered through the Health Disparities Collabroative. This process is used to continually improve each patient's care management and the overall quaity of care received.

The model implemented is part of the Health Resources and Services Administration efforts to improve clinical quality. While initially created for use in chronic disease management, the Care Model and the Model for Improvement has been expanded into other areas of community health centers as well.

Chronic disease patients are entered into a database registry and providers are able to follow-up and monitor scheduled care to maintain steady managment and prevent a health care crisis before it starts.
